- 요약 -Nifedipine(Nipine??)은 Ca++통로 차단제로서 혈관의 평활근에 작용하여 Ca++의 유입을 차단하고 시신경의 허혈성 변화로 인한 시야결손이 특징인 저안압녹내장 환자에게 투여시 혈관을 확장시켜 혈류공급을 원활하게 하여 시야를 개선시킨다고 알려져 있다.저자들은 저안압녹내장 환자 7명 11안을 대상으로 2주 이상 Nifedipine 10mg을 1일 3회경구 투여한 후 최소한 3번 이상의 Humphyrey(Allergan, Statpac)자동시야계를 이용하여 망막의 광감도를 측정하였고, 각 Sector에 따른 광감도의 변화 및 Global index의 변화를 조사하여 Student Paaired t-test로 비교하였다.Nifedipine 복용 후 시야의 광감도의 변화는 11안중 7안에서 광감도의 증가를 보였으며 이중 4안이 통계적으로 유의한 차이를 보였다. Nipine복용한 환자의 시야에서 상비측이 다른 부위보다 큰 폭으로 광감도가 증가하였고 Mean deviation은 개선되었으나 그 유의성은 없었다. It is known that Nipine○R improve the visual field by increasing blood supply to the optic nerve with dilatation of vessels in low tension glaucoma patient whose visual field loss is thought to be caused by the ischemic change of optic nerve. We have clinically evaluated the sensitivities of visual field of 7 cases(11 eyes) of low tension glaucoma, who were medicated with Nipine○R, 10mg Tid per os for more than 2 weeks. The visual field were tested at least 3 times or more with Humphrey automatic perimetry (Allergan, Statpac) and were divided into 10 sectors according to the nerve fiber bundles. In each sector, we analyzed the change of light sensitivity and global index, then compared by Student Pared t-Test. After medication of Nipine○R, improvements of mean light sensitivity in total sector were found in 7 eyes among the 11 eyea and also superonasal portion of their field was more recovered than the other portions and they showed the improvement of the mean deviation but it was not significant(J Korean Ophthalmol Soc 33: 871~877, 1992).
