= 요 약 =저안압 녹내장은 21mmHg 이하의 정산안압에서도 시신경 유두변화 및 시야변화를 일으키며 우각경 검사상 광우각인 녹내장이다. 시야검사에서 나타는 암점은 고안압 녹내장 환자에서 보다 그 경사가 급하여 주시점이 가깝고 또 깊이가 깊다고 하였다.저자들은 저안압 녹내장에서 안압의 하강정도가 시야개선에 미치는 영향을 알아보기 위해 1990년 5월부터 1992년 12월까지 본원 안과에 내원한 저안압 녹내장 환자 18명 36안을 대상으로 베타 차단제 등의 약물치료전 안압과 치료후 안압을 비교하여 안압이 30%이상 감소한 A군과 그러지 못한 B군으로 나누어 Humphrey 자동시야계(Statpac)를 이용하여 검사한 시야검사 결과를 비교분석하였고 그 상관관계를 unpaired t-Test로 검정하였다.두군간의 global indices를 비교한 결과 mean defect(MD)만이 통계적으로 유의 (p<0.05)한 차이를 보였으며 MD의 경우 치료전에 비해 A군에서는 평균 49.89% 감소하였으나 B군에서는 오히려 1.1% 증가하였다. 이상의 결과로 저안압 녹내장에서 적어도 30%이상의 안압을 떨어뜨려야 MD의 개선을 유도할 수 있다(한안지 35:1663~1668, 1994).In low tension glaucoma(LTG) , patients have changes such as glaucomatous atrophy, cupping, and excavation of the optic disc and typical glaucomatous visual field defect, yet their intraocular pressure(IOP) is normal.We performed a retrospective study on the change of visual filed global indices of LTG depending on the reduction rate of IOP among 36 eyes of 18 patients from 1990 to 1992.All eyes were treated medically and the visual field was tested with Humphrey automatic perimetry(Allergan, Statpac).Patients were divided into two group according to the IOP reduction rate(group A<30% vs group B≥30%). The effect of magnitude of IOP reduction on the improvement of global indices were proved by comparing the global indices between two group using the unpaired t-Test. Only the mean of mean defect(MD) improved in group A compared to group B(p<0.05). This study demonstrates a quantitative evidence of improvement of MD according to IOP reduction in patients with low tension glaucoma, and underscores the beneficial effect of a greater magnitude of IOP reduction(J Korean Ophthalmol Soc 35:1663~1668, 1994)
